Out-of-district transfer information and form - If you live outside the White River School District boundaries, but wish to attend a school in White River School District, you must submit this form on an annual basis. Obtain a Release of Attendance from the school district in which you live and attach it to this form.
Elementary Intra-district transfer information and form - If you live within the White River School District boundaries, but wish to tranfer to a White River Elementary School outside your attendance area, you must submit this form on an annual basis.
Release of Attendance Form - If you live within the White River School District boundaries, but wish to attend a school outside our district, you must submit this form on an annual basis and have it signed by the White River Superintendent.
Transcript Request Form- Use this form to request a High School Transcript from White River High School or Collins High School. Fax or Bring this signed form and picture ID to the Superintendent's Office.
Authorization for Administration of Medication at School - Complete this form if your child will need to have any kind of medication given at school. Turn it in to the school office.
