An important message regarding immunizations:
            

Please remember that in order for your student to obtain their class schedule, immunizations must be current, or a Certificate of Exemption signed by your physician must be on file.  For your reference, as provided by the Tacoma - Pierce County Health Department, the "Minimum Immunizations Required for School Attendance Reference Sheet for Parents and School Staff By Grade Level, 2015-16  can be found  at this link.

Ebola Virus Information
We have had no cases in the Pacific Northwest. Our national health system has the capacity and expertise to quickly detect and contain this disease and is working with states and school districts to ensure the safety of our students and school employees.  As you likely know, the CDC is continually updating its information on Ebola, information that can be found here: http://www.cdc.gov/vhf/ebola/index.html.

Please note: Flu is a much greater risk in our area, according to the Tacoma-Pierce County Health Department. Nationwide, the flu kills 11,000 annually, and we had 10 flu-related deaths in Pierce County during last flu season.

The 2016-17  School Year

If your child will require an individualized health care plan or medications during their school day, please find below the forms that will be required 

Individualized Health Care Plan / Emergency Care Plan:   You will find that you can print a copy of your child's existing Individualized Health Care Plan or Emergency Care Plan for bringing with you to your Health Care Practitioner  for updating for the next school year,  through your Family Access on the district website  or,  you can print a blank  copy from the link below.

Medication In School:     White River School District #416 Board Policy No. 3416 addresses Medication at School.   This policy is in accordance with Washington State Law and specifies the conditions by which medications are allowed to be given to students.  The required "Physicians Orders for Medication in School" can be found in the Health Services Student Forms links below.  

Student Immunizations:
White River School District #416 Board Policy No. 3413 addresses Student Immunizations. This policy is in accordance with Washington State Law and addresses information specific to immunization requirements.   The Washington Department of Health Certificate of Immunization Status, required on enrollment and referenced in the district's board policy can be found in the Health Services Student Forms links below.
