Out-of-district transfer information and form- If you live outside the White River School District boundaries, but wish to attend a school in White River School District, you must submit this form on an annual basis. File a Choice Transfer Request with the school
di​strict in which you live.
 
Parents may complete a Choice Transfer Request through the use of the new Choice Transfer Request Portal.  This will save you a trip to your resident school district! (A few districts do not use this option. If you do not see your resident district listed within the link, you will need to obtain a release from your resident district directly.)  Please note, this link works best using Internet Explorer as your browser.
 
Elementary Intra-district transfer information and form- If you live within the White River School District boundaries, but wish to transfer to a White River Elementary School outside your attendance area, you must submit this form on an annual basis.
Choice Transfer Request- If you live within the White River School District boundaries, but wish to attend a school outside our district, or enroll in an on-line program through another school district, you must complete this form annually.

Transcript Request Form- Use this form to request a High School Transcript from White River High School or Collins High School. Fax, mail, e-mail, or bring this signed form and picture ID to the Superintendent's Office.
WRSD - Harassment, Intimidation or Bullying (HIB) Incident Reporting Form:
- Harassment, intimidation or bullying is an intentional electronic, written, verbal, or physical act that: Physically harms a student or damages the student’s property. Has the effect of substantially interfering with a student’s education. Is so severe, persistent, or pervasive that it creates an intimidating or threatening educational environment. Has the effect of substantially disrupting the orderly operation of the school.
