Vendors

First time doing business with us? The White River School District makes it easy to become a new vendor!  We require an up-to-date W-9, and a Consultant/Contractor Agreement (if applicable) . In addition, for certain types of vendors, we are required to collect information to perform a Washington State Department of Retirement System Member Verification. 

 

Please submit your completed W-9 and Consultant Agreement to the Business Office via Email or fax: 360-829-3843. We will contact you if we require additional information. We have also included an Invoice template below for your convenience.
